EXPERIENCE

Melanie Holt Fauth, BSN is a visionary senior-executive with expertise in health system strategy and operations, including program development for senior care and post-acute networks.

Throughout her career, Melanie has been repeatedly recognized for her achievement in the planning and execution of post-acute networks and programming across a large state-wide health system, and for the development and growth of a long-term acute care specialty hospital.

Selected accomplishments include an integrated health system strategy to optimize post-acute and senior care across a 19-hospital system and large medical group. In addition, she led the business development and creation of post-acute networks with 50 unique entities, across an entire care continuum, to address key patient needs for LTAC, inpatient rehab, skilled nursing, home care, hospice, and palliative care. Melanie also led efforts to integrate programming such as remote patient monitoring and high acuity specialty clinics into care pathways to reduce hospital readmission rates.

As a registered nurse for 32 years, Melanie continues to leverage first-hand experience with patient care and operations to identify the best interests of all parties when adopting new business models or reevaluating their approach to value-based care. She has a proven track record of surpassing goals and identifying partners and key stakeholders to meet budgets and deadlines across complex, matrixed organizations.

Melanie has also owned and managed financial and operational aspects of multiple hospital departments, honing her eye for health system growth and budgeting.

Melanie is a member of the Indiana Geriatrics Society and Indiana Executive Women in Healthcare. She earned her BSN/BA in Nursing and Business at DePauw University, Indiana and is a 2019 Tobias Fellow through Indiana University.
